Family Service Society of Yonkers (FSSY) is a community-based nonprofit organization that has served residents of Yonkers and the surrounding areas of Westchester and the Bronx for over 130 years. We provide comprehensive services for individuals and families of diverse backgrounds and ages, including the largest and most highly regarded Guardianship program in the region.

FSSY has an urgent need for a full-time Case Manager to join our Guardianship program. As part of a unit of social work professionals and under the supervision and training of a LMSW and senior case management team, you will be responsible for:

· Coordinating services for clients, some of whom are disabled and or elderly with a diagnosis of cognitive deficits, dementia or Alzheimer’s disease;

· Working with family members, the court system, social services agencies, community resource programs, hospitals, nursing homes and other elder care facilities;

· Spending approximately 50% of your time in the field visiting your clients in facilities or in their home environments.

Requirements:

· Bachelors’ degree in Social Work or Social Services; Bachelor’s degree in Human Services, Counseling or a related discipline;

· Bi-lingual (English/Spanish) desirable;

· Computer competency, including Microsoft Office suite

· Possession of a valid Driver’s License;

· Access to an automobile for client visits
