FAMILY SERVICE LEAGUE is Hiring a LICENSED PRACTICAL NURSE Near Riverhead, NY

SUMMARY

Family Service League is seeking a full-time Licensed Practical Nurse (LPN) for our Certified Community Behavioral Health Clinic (CCBHC) in Riverhead. The LPN will serve as a part of the multidisciplinary team. The LPN will be responsible for health monitoring services, tracking prior authorizations and administration of intramuscular injections of psychotropic medication under direct supervision of a Registered Nurse or other assigned supervisor.

RESPONSIBILITIES

  • The LPN will conduct health screenings, monitor vital signs and side effects of medications, and report findings to the care team. 
  • Assist with nursing functions as specified by program (i.e. IM injections, medication line, prior authorizations)
  • The LPN will conduct urine toxicology screenings.
  • Complete progress notes and maintain nursing and health related data in the electronic medical record (EMR).
  • Maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner, as per policy and procedure.
  • The LPN will maintain proper boundaries with clients.
  • Fully understand the role and expectations within the program.
  • All other duties as assigned.

QUALIFICATIONS 

High school diploma or equivalent required.

Licensed Practical Nurse (LPN) with current New York State license required. 

Current CPR Certification required.

Minimum of 1 year of related experience in a psychiatric and/or SUD setting required.

Proficient computer skills and experience with Electronic Health Records required.

Excellent interpersonal, verbal, and written communication skills required. 

Bilingual in Spanish strongly preferred.
