We have a full time (40 hours per week) The hours are Monday through Friday, 8:00 a.m. - 5:00 p.m. Care Coordinator position in our Long Term Services and Supports (LTSS) Community Partner (CP) program in the Cape Cod region. This position is partially remote with 2 drop in days in Mashpee office in Cape Cod area, and includes travel to Outer/Lower Cape Cod with occasional travel to the Fall River corporate office.
The Care Coordinator is responsible for promoting coordination in the delivery and receipt of LTSS to enrollees assigned by Accountable Care Organizations (ACO) and Managed Care Organizations (MCO).
