TITLE: Site Coordinator - LAP

FT/PT:
Full time (35 hours per week, may include some evenings and weekends)

CLASSIFICATION:
Salaried – Non-Exempt

PAY GROUP:
Grade 18

REPORTS TO:
Program Director

SUPERVISES:
Advocates

FUNCTION:
The Site Coordinatorhas the responsibility to provide assistance and supervision in the Advocates. Coordinate with off-site locale to create opportunity to refer identified clients to appropriate services. Additionally direct services to victims of domestic violence will be provided.

ESSENTIAL DUTIES & RESPONSIBILITIES:
  • Directly supervises Advocates, volunteers and interns, providing them with adequate support and feed back to carry out the program directives, and maintain supporting documentation.
  • Provide direct services to clients in both individual and group setting.
  • Make referrals to identified clients as required or needed.
  • Maintain relationships with external partner agencies.
  • Assist in scheduling of staffing patterns to meet client and program needs.
  • Carry out the program directives, and maintain supporting documentation.
  • Participates in the orientation of interns and volunteers.
  • Will conduct performance evaluations of advocates, interns and or volunteers.
  • Will ensure that intakes and orientations are completed, and carry out the completion of these activities as necessary.
  • May develop Service Plans jointly with each client.
  • Will continuously monitor progress of the client(s), and adjust service plans as necessary. Goals and objectives as well as time frames must be clearly specified.
  • On behalf of the client and/or Family Rescue, will coordinate and advocate with outside entities which may include; social service organizations, mental health organizations, substance abuse organizations, housing organizations and/or legal/law enforcements agencies, etc.
  • Provide effective case management/documentation which includes completing all required documentation in a timely manner and maintaining records, files, reports etc. as directed.
  • Conduct quarterly reviews of case files in coordination with the Program Director and or Program Assistant.
  • Monitor and review client files.
  • Provide follow-up service to clients once they have left Family Rescue services.
  • Attend staff meetings, trainings etc. as required and/or scheduled.
  • Other Duties as assigned.

MARGINAL DUTIES & RESPONSIBILITIES:
  • May assist in picking up and distribution of donations.
  • May do research and/or gather resources to directly assist clients and/or to perform job.
  • May assist in the up-keep (cleaning) of office/building space(s).
  • May provide assistance with transportation to scheduled appointments.
  • May assist in other programs both inter-agency as well as inter-program.
  • Responsible for new staff orientation.

QUALIFICATIONS:
Education & Experience:
  • Prefer a B.A. degree in social service or related field with one (1) year of applicable experience.
  • Will consider candidate that has 4 years related work experience.
  • Must have one (1) year supervisory experience.
  • Must have 40-hour domestic violence training (within 3 months of hire date).
  • Must be Illinois Certified Domestic Violence Professional (within 1 year of hire date).
  • Must be CPR/first aid certified (within 1 year of hire date).

Language/Writing Skills:
  • Must have excellent verbal skills.
  • Must have the ability to communicate effectively to diverse group(s) of people in varied settings.
  • Must be able to write in a logical, clear and grammatically correct way.
  • Bi-lingual Spanish speaking, reading and writing may be required.
