Family Promise of Sussex County is Hiring a Case Manager Near Newton, NJ

Family Promise of Sussex County is looking to hire an experienced case manager to conduct home visits with clients in the supportive housing program. The Case Manager will work with chronically homeless individuals in Sussex, Warren, and Hunterdon Counties to enter a permanent supportive housing (PSH) program, gain access to resources, and create long-term housing stability plans. The worker will provide advocacy, support, educational resources, and referrals through one-on-one case management and group sessions.

The staff member is responsible for providing intensive case management services, financial and career counseling, life skill seminars, budgeting, and other relevant training sessions to meet the household's needs. Documentation is to be maintained for all program participants in the online data management system.

Responsibilities include, but are not limited to:

  • Complete, intake, assessment, referral packets, and other paperwork as needed to support the client in accessing housing resources
  • Collaborate closely with other staff and outside agencies in meeting client needs
  • Establishes rapport with clients and obtain and document the essential data as required by program funders and agency
  • Engage the services of collaborative agencies to meet client household needs
  • Assist clients in scheduling and maintain appointments for medical visits, housing search, applications for benefits and other resources as needed
  • Complete Prioritization Tool and enter client data to waitlist for availability of PSH units
  • Attend Continuum of Care Case Conference meetings and assist with the facilitation of case review
  • Enforce agency policy and procedural rules and regulations of the program to ensure safety for both client and employees
  • Remain current on knowledge of area resources relevant to agency and client needs
  • Data input and maintenance utilizing the Homeless Management Information System (HMIS) database and other spreadsheets as needed
  • Utilizes appropriate crisis intervention techniques for client stabilization
  • Accurately assesses risk to self, others or property and the presence of a grave disability
  • Develops and documents appropriate safety plan
  • Provide ongoing advocacy, support, and services to maintain stabilization of households of clients
  • Create monthly, quarterly, and annual reports for program outcomes, annual performance reports, program monitoring, and others as required by administration and grant funder
  • Completes all required progress notes according to agency comprehensive guidelines, policies and procedures regarding format, content and timeliness.

Requirements

  • Minimum Bachelor Level with 3 year experience working with chronically homeless, disabled, and low-income populations. Master Level, LSW or LCSW preferred
  • Must have a valid driver's license in good standing

Job Type: Full-time

Pay: From $55,000.00 per year
