Family Promise of Hawaii is Hiring a Disaster Case Manager Near Maui, HI

Job Summary

The Disaster Case Manager (DCM) works directly with individuals and households affected by the August 2023 Maui wildfires, acting as a facilitator and advocate to empower their recovery process. The DCM helps impacted families identify unmet needs and determine recovery goals, then collaborates with other agencies to meet those needs and goals, while ensuring the family is empowered to lead their own journey to recovery. A team of eight DCMs reports to the Disaster Case Management Supervisor. DCMs are full-time, long-term positions and candidates must be based on Maui.

Family Promise of Hawaiʻi is a 501(c)3 nonprofit that provides critical housing and wraparound services for families experiencing housing instability and homelessness. Our mission is to help homeless and low-income families in Hawaiʻi achieve sustainable independence by mobilizing existing community resources and support. We provide emergency shelter, rental assistance, prevention/diversion services, and holistic case management.

Essential Job Functions

Intake

  • Perform outreach, receive referrals from Family Promise of Hawaiʻi’s phone line and community partners, and promptly follow up with families.
  • Lead the intake process for all potential families, help families review all that has happened and all the assistance they have received, and begin identifying unmet needs, through a comprehensive, individualized, strengths-based, and culturally responsive approach.
  • Communicate with other staff and partners about any necessary family information.

Recovery Support and Advocacy

  • Work alongside families to identify their recovery goals.
  • Facilitate trauma-informed case management sessions with families to discuss unmet needs, including housing, financial support, physical and mental health, and other areas.
  • Identify the level of support that families need to accomplish their recovery goals and support families throughout the process, acting as a liaison and advocate to resources as needed.
  • Provide crisis support to families as needed.

Resources and Referrals

  • Connect families with any necessary community resources, such as housing, financial resources, employment support, child care, counseling and mental health care, and educational programs.
  • As needed, work with other staff to deliver resources, referrals, and workshops related to survivors’ unmet needs.
  • Serve as an advocate for families with community service agencies and network with other agencies to stay informed about resources and services.
  • Leverage the organization’s resources and partnerships: Coordinate with partner organizations and help to maintain the organization’s internal list of resources for families.

Administration

  • Maintain case file records for every family, including documenting all case management activities and services provided.
  • Input family data into organizational databases.
  • Complete exit procedure including exit forms and exit survey.
  • Attend training sessions to learn to identify, validate, and collaborate on solutions to meet survivor needs.
  • Additional responsibilities as needed.

Required Qualifications & Experience

  • 4-year degree in human services field or equivalent experience.
  • Self-motivated and passionate about supporting the recovery efforts on Maui.
  • Empathetic and able to empower families’ choices.
  • Detail-oriented, with solid communication and presentation skills.
  • Team player who works well with a variety of personalities.
  • Able to accept feedback and adaptable to change.
  • Flexible with work schedule.
  • Valid driver’s license and reliable transportation.

Preferred Qualifications & Experience

  • Fluent in another language, particularly Pacific Islander languages.
  • Prior professional experience working in the human services field.
  • 4-year degree or master’s degree in social work.

Compensation and Benefits

The salary for this full-time position is $55,000 – $60,000.
