Family Practice Counseling Network is Hiring a Care Coordinator/PSR (FT) Near Philadelphia, PA

Position Summary:
The Family Planning School-Based Health Center (SBHC) Care Coordinator/PSR works directly with the SBHC NP to provide comprehensive family planning services to high school students. Services include: contraception counseling and prescriptions, sexually transmitted infection testing and treatment. The SBHC will include an Integrated Behavioral Health Consultant (BHC) and the Health Resource Coordinators (HRCs).
Team members are expected to uphold the health center mission by assuring that patients receive health care that is competent, caring, and meets with a high degree of satisfaction. In addition, staff must support FPCN's commitment to the creation of a trauma-informed system of care that continually recognizes and responds to the impact of traumatic stress on all those who have contact with the organization, including children, adults, families, caregivers, and staff.
Essential Duties and Functions:
  • Assist in the courteous and efficient management of connecting students with the appropriate services and/or staff in a timely and accurate manner.
  • Initiate services for patients which involve signing in, updating basic demographic information, validating health insurance coverage, and preparing the necessary paperwork. Additional services may be warranted depending on if the patient’s status is new or established and if the patient seeks more specialized services.
  • Schedule appointments for patients.
  • Coordinate with the school to bring students in for their appointments.
  • Constantly update and utilize pertinent information about the health insurance carriers that the Practice accepts.
  • Maintain patient charts.
  • Provide additional clerical tasks: filing, typing, faxing, and copying documents.
  • Facilitate the ordering of office/janitorial/medical supplies for the respective site.
  • Maintain patient confidentiality in both written and verbal formats.
  • Possess exemplary knowledge of and enforce all Practice policies and procedures that impact the work of the PSR and front desk. 
  • Adhere to OSHA guidelines.
  • Assist in coordinating utilization of the Practice’s transportation services for students needing appointment at one of FPCN’s clinics.
  • Take primary responsibility for securing the clinic space at the close of the business day.
ESSENTIAL DUTIES ADDENDUM 
Care Coordination: 
  • Be a member of the multidisciplinary team consisting of a Nurse Practitioner and Behavioral Health Consultant. 
  • Manage referrals to other services both inside and outside of FPCN.
  • Liaison with Health Resource Coordinators.
  • Insurance coordination- work with FPCN Outreach Staff to assure patients receives appropriate benefits.
  • Coordinate all components of primary care, outreach and the HIV and High-risk patient care (e.g., scheduling appointments, performing appointment reminder activities, conducting follow-up calls for no-shows, troubleshooting non-clinical patient issues).
  • Track laboratory tests to ensure completion.
  • Establish trust relationships with patients and communicate concerns to appropriate clinical and school staff.
Qualifications:
  • Certified Medical Assistant or Licensed Practical Nurse. 
  • One (1) year in a Health facility.
  • Previous experience with an Electronic Health Record. 
  • High School Diploma/GED.
  • Computer Competency with proficiency in word processing.
  • Ability to travel to multiple sites (2-3).
  • Desire to work with high school students and comfortability discussing sexual health topics.
  • Exemplary skill at managing multiple tasks simultaneously.
  • High level of patience, courtesy, and concentration.
  • Outstanding interpersonal skills.
  • Outstanding verbal and written communication skills as well as listening skills.
  • Exemplary organizational skills and ability to prioritize.
  • Highly reliable and dependable.
  • Experience in effectively relating to people.
  • Ability to effectively relate via telephone.
  • Responds constructively to pressure situations and time limits.
  • Initiatives work independently with minimum amount of supervision. 
  • Exemplary Current CPR.
