Seeking an experienced and dedicated RN Case Manager for a patient/family centered hospice team. Assistant PCC/ Admissions Position is a salary position including benefits & mileage. Position offers rewarding atmosphere and nursing autonomy with 1-1 bedside care. Candidate must possess critical thinking and good communication skills. The ideal RN candidate has at least 1 year of Hospice or Home Health experience. This position works Monday through Friday 8:30 - 4:30. On-call back- up is required in rotation with other management staff. Any patient care hours worked on-call are paid hours beyond fixed salary.
