Family Home Health / Three Rivers Hospice is Hiring a Hospice Social Worker Near Butler, PA
Three Rivers Hospice, a division of Quality Life Services is seeking a Full-Time Social Worker in the BUTLER and ARMSTRONG COUNTY areas who wants to make a difference in the lives of older adults. Essential Functions, include but not limited to:
- Assess the psychosocial status of patients / families / caregivers related to the patient’s terminal illness and environment
- Provide an assessment in the patient’s identified residence and assistance with this is not safe and another plan is required
- Carry out social evaluations and plan interventions based on evaluation findings
- Counsel patient / family / caregivers as needed in relationship to stress and other identified coping difficulties
- Maintain clinical record on all patients referred to social work
- Oversees and instructs medical social worker interns per specific college requirements
- Maintain collaborative relationships with organization personnel to support patient care

- Master's Degree in Social Work (MSW)
- Provides services for all hospice patients
