Family Health Centers of San Diego is Hiring a Remote PACE Dietician

Job Roles Perform initial screening to develop care plans and monitor participant’s progress. Promote utilization of nutritional counseling and education through case finding techniques to promote the growth of the nutrition services within the PACE Program. Evaluate participant’s nutritional status through use of various lab values and physical assessments, and obtaining primary care provider’s orders as required. Assess participant’s food preferences, methods of preparation and cultural and environmental factors affecting the participant’s dietary intake. Participate in the development and revision of the participant’s plan of care as a member of the interdisciplinary team. Develop a plan for the provision of food and drink appropriate for the participant’s physical and medical needs and environmental conditions. Integrate the nutrition treatment plan into the overall plan of care developed by the interdisciplinary team. Educate the participant’s family or other caregivers about the need for and preparation of therapeutic diets as part of the treatment plan. Re-evaluate and revise nutritional plans as necessary to ensure the continued adequacy and appropriateness of the nutritional plan, obtaining primary care provider’s orders as necessary. Maintain accurate records and report client’s treatment response to the team and primary care provider. Perform other duties as assigned. Education/Certifications/Licenses/Registrations Bachelor’s Degree in Dietetics or closely related field required. National Certification by Commission on Dietetic Registration required. Food Handler Permit required, and Safe Serve certification highly desired. Ability and means to travel as needed in a timely manner within San Diego County, to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Age 21; Licensed for a minimum of 3 years; No violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless or felony Driving within 7 years. Experience/Specialized skills (including Language) 2 – 4 years of experience in nutrition and/or chronic disease education or closely related field required. Experience working with the older adult population preferred. Demonstrated knowledge in the principles and practices of nutritional counseling and education. Demonstrated ability to work effectively with social, cultural and economically diverse populations. Demonstrated ability to be culturally sensitive and respect diversity. Ability to maintain good interpersonal interactions with participants and co-workers as a member of a multi-disciplinary health care team. Excellent written and verbal communication skills. Knowledge of current nutritional science theories and best practices with the ability to translate into meaningful and culturally appropriate resources. Knowledge of education techniques, practices, and tools for population served. Knowledge of community resources, health and social service systems in San Diego County to establish working relationships with community partners. Basic computer literacy ability to comply with department needs and expectations. Strong interpersonal and customer service skills. Bilingual in Spanish/English preferred. In addition to these factors – we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
