This is a 24 hour onsite position to support the OBAT Team.

The Registered Nurse is an integral part of the multidisciplinary care team responsible for ensuring that the primary care provider and the practice team maintains a central role in the care process, by coordinating the care of these vulnerable patients and ensuring the patients receive optimal care including acute illness management, chronic disease management, self management coaching, and wellness. preventative care as they transition across multiple health settings and/or multiple providers.

 Educational Requirements:

Graduate of accredited school of nursing required; AS or BS in nursing preferred. 

Experience Required:

Two years full time professional experience or part time equivalent 

Professional Licensure/Special Skills And Certification Required:

Must be licensed as a RN by the Massachusetts State Board of Registration. BLS required. 

Supervisory Responsibilities:

Precept medical assistant staff by assisting and providing direction with procedures and activities that facilitate patient centered care.

Reporting Relationship:

Reports directly to the Nurse Team Leader, or at Satellite Sites, reports directly to the RN Coordinator Satellite Site.
