Family Health Center is Hiring a Registered Nurse - HIV Team Near Worcester, MA

 

Summary: 

The LPN or RN Case Manager nurse for the HIV team will work effectively as an interdisciplinary team member dedicated to delivering high quality HIV services with a strong commitment to providing accessible care to HIV positive patients facing challenges such as: poverty, mental illness, addiction, homelessness, and other barriers to healthcare.

The HIV nurse will follow Ryan White Grant guidelines and provide medical case management to a growing caseload of adult patients in a family practice setting. The nurse will participate in all aspects of patient care and case management including triage, HIV patient education, and long-term follow up, as well as support the HIV Medical providers in the administration of vaccines, medication, and various nursing duties. 

 EDUCATIONAL REQUIREMENTS: 

Graduate of an accredited school of nursing. Required AS or BS in nursing preferred. 

Must be a licensed LPN or RN by the Massachusetts State Board of Registration.

EXERIENCE REQUIRED: 

Two years full-time professional experience or part-time equivalent. 

INTERPERSONAL RELATIONSHIPS/ORGANIZATIONAL EXPECTATIONS: 

  1. Commitment to providing excellent health care/case management to HIV adult patients in a medically underserved community.
  2. Maintain strict confidentiality dealing with sensitive information.
  3. Effective communication skills, both oral & written, with patients, families, employers, coworkers, health care professionals, funders, and other regulatory agencies.
  4. Strong computer skills, including use of an electronic medical record.
  5. Ability to work independently and within a team environment with minimal supervision, and to plan, coordinate, and complete tasks necessary to meet job responsibilities. 
  6. Awareness of both internal and external resources that accommodate the needs of patients. 
  7. Providing fiscally responsible, timely, efficient, effective, and safe patient-centered, culturally competent, equitable, and participatory health care and case management.
  8. Processes personal health information in accordance with FHCW HIPPA policies & procedures. 
  9. Conduct initial assessments for eligibility and determination of patient needs/goals, and subsequently provide effective, ongoing case management in the medical tracking/following of patient caseload.
  10. Perform occasional home visits as needed.
