Essential Duties and Responsibilities

• Maintains working knowledge of and adhere to the program Manual, agency handbook and Policies and Procedures

• Commitment to providing cost conscious, fiscally responsible, timely, efficient, effective, safe, patient-centered, culturally competent, equitable, and participatory health care.

• Attends all required trainings as per Agency Policy, Program Manual and Funder Guidelines.

• Arrives on time for work; remains at work for whole shift; regular and predicable attendance.

• Maintains appropriate boundaries and ethical standards with staff, clients, visitors and outside providers as detailed in Agency Policy.

• Maintains positive and professional working relationships with co-workers, managers, clients, visitors and outside providers 

• Strong and clear written and verbal communication skills

• Excellent computer skills (included but not limited to Microsoft Word, Excel and Outlook)

Program Specific Essential Duties and Responsibilities:

• Treats individuals of all ages and will provide culturally competent care, utilizing interpretation services when indicated. 

• Engages in and supports productivity in order to ensure access to the clients seeking services. 

• Completes biopsychosocial assessments. 

• Completes treatment plans in conjunction with clients. 

• Provides appropriate counseling/therapy to clients.

• Works with medical providers to formulate cross-disciplinary treatment plans for complex cases.

• During initial session, explains services, rights and obtains client agreement by completing initial evaluation after three sessions.

• Keeps abreast of all insurance requirements and submits paperwork accordingly.

• Continually evaluates service and maintains a productivity level that adequately meets the goals of the department.

• Works directly with medical providers to collaboratively address clients/patients’ needs and to support optimal medical and behavioral health.

• Any and all other duties as assigned. 

Qualifications and Education Requirements

One of the following combinations of education and employment experience must be met in order to be considered for the position:

• Professional Licensure/Special Skills and Certification Required: Candidate has an active LICSW license.
