Family Health Care of Siouxland is looking for a full-time Psychiatric Nurse Practitioner to join the team!
LOCATION: Family Health Care of Siouxland – Sioux City, IA
GENERAL SUMMARY OF DUTIES: Independently works with staff to provide outpatient psychiatric care including identification, diagnosis, treatment, medication management, in collaboration with primary care physicians.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Provides psychiatric care, medication assessments, and crisis response to new and existing patients. Develops individual care plans based upon assessment data, and provides therapeutic interventions as necessary, including the provision of on-going medication management services.
2. Participate in team meetings as needed.
3. Review and assess referrals for medical and psychiatric appropriateness in collaboration with the prescriber, program manager and clinic director.
4. Provide medical and psychiatric education to clients and their families.
5. Conducts medical/health/wellness assessments to identify health care needs and assist in developing health care goals with the treatment team.
6. Collaborate and coordinate with primary and specialty care providers.
7. Graduation from an accredited Psychiatric Nurse Practitioner Program required.
