Location: Palm Bay, Melbourne, Titusville, Meritt Island

The Family Empowerment Project with support from the Healthy Start Coalition of Brevard County is dedicated to improving maternal health and well-being through impactful health education and compassionate care coordination. We strive to ensure that every mother receives the support and resources she needs to navigate the journey of motherhood with dignity and resilience. Together with the Coalition, we strive to empower mothers and their families through their journey to motherhood and improve health outcomes for babies born in Brevard County by supporting, educating, and connecting mothers to the resources they need.

The Healthy Start Home Visitor is responsible for providing Healthy Start participants counseling to address emotional, situational, and developmental stressors. The goal is to reduce identified risk factors to achieve positive pregnancy outcomes and optimal infant/child health and development. Participants are screened and assessed, and provided with care coordination (connection to services), health education, and referral/ follow-up to pregnant women and infants in the Healthy Start Program. The Healthy Start Home Visitor exercises individual judgment and works with minimum supervision to empower women to improve the health of themselves, their infants, and their families.

: Minimum of four-year college degree in one of the following preferred:

  • social sciences; a health-related field such as nursing, public health, health education, health care administration, social work or psychology

-OR-

  • Two years of college with three years of experience in social work, public health, health education, nursing, or counseling.
  • Bilingual in Spanish or Creole preferred.
  • Reliable transportation is required for client home visits.
  • Computer skills with proficiency in Microsoft Office (Excel, Word, Outlook) and the internet.
  • Ability to interface effectively with a diverse population.
  • Professional communication, documentation, and time management skills.
  • Must communicate effectively both verbally and in writing.
  • Ability to be flexible regarding work location and work schedule, occasionally including evenings or weekends outside of regular business hours, based on business needs.
  • Bilingual preferred.
  • Excellent oral and written communication skills.
  • Must possess strong organizational, communication and writing skills.
  • Bilingual in Spanish or Creole preferred.
  • Home visitors will be fluent in reading, writing, and translating information on all levels.
  • Relate cooperatively and constructively with staff, agency representatives, service providers, and the priority population.
  • Deliver evidence-based, prenatal, preconception, and inter-conception health education programming to identify clients at hospitals, community health clinics, and in their homes as outlined in program guidelines.
  • Coordinate health screenings to assess the educational needs of clients at- risk for poor pregnancy outcomes and provide individual counseling to improve the health & well-being of clients.
  • Assure clients and their families have access to accurate and up-to-date health information and care.
  • Perform case management services for individuals/families receiving Healthy Start services.
  • Assist clients in accessing community services and provide information and education to clients about smoking cessation, childbirth education, breastfeeding support, parenting and infant growth and development.
  • Initiate scheduling procedures for client, family of client and client’s provider to manage appointments and referrals for services.
  • Complete all Healthy Start assessments and home visits as required per program guidelines.
  • Maintain/update client files and ensure Healthy Start database are updated as required per policy and program guidelines.
  • Conduct outreach activities including home visits to clients to provide Healthy Start services and to providers and other community organizations to promote the program.
  • Participate in quality improvement and quality assurance activities of the program to enhance services.
  • Prepare and facilitate maternal group connections as required.
  • Complete documentation and accurately code for Healthy Start services in Well Family System within 72 business hours.
  • Complete initial contacts as required by the program guidelines and core performance measures.
  • Serve as a liaison and/or participate in county and community agency meetings that involve strategic planning and implementation of public health programs that benefit priority population as assigned.
  • Assists with completion of quarterly reports.
  • Build relationships with community organizations, local health departments, and community based clinic practices to better serve clientele.
  • Participate in staff meetings, continuing education training, and program required meetings.
  • Complete all job duties and case management tasks within the required times.
  • Comply with HIPAA requirements regarding client confidentiality as they pertain to the Coalition and the Treasure Coast Maternity Center
  • Work on special projects or tasks as assigned.
  • Perform duties in a culturally sensitive manner.
  • Requires flexible work hours as necessary
  • Maintain office calendar with all daily activities, meetings, and work tasks
  • Completes and submits mileage forms according to designated timelines.
  • Attends and participates in weekly all-staff meetings.
  • Attend meetings, classes/conferences, and in-service programs as requested or assigned to update knowledge and skills related to the Healthy Start Program and other activities.
  • The ability to function and operate as a team player, showing dignity and respect for all.

Job Type: Full-time

Pay: Starting rate $18.00-$18.50 per hour. Rate is evaluated at 90 days of employment. Increases are based on performance measures and availability of funding.
