As a HomeSafe Therapist/Clinician, you will play a vital role in supporting families dealing with stressors like homelessness, substance abuse, mental health concerns, and child separation due to these stressors. Your responsibilities include:

๐Ÿš€ Programmatic and Service Model Mastery:

  • ๐Ÿ“ž Prompt Client Outreach: Respond promptly to client requests for phone screens and intake assignments.

  • ๐Ÿ“Š Accurate Assessment: Assess clients' presentations, needs, and strengths accurately.

  • ๐Ÿ“‹ Timely Intake Assessments: Complete intake assessments promptly.

  • ๐Ÿค Engagement, Planning, and Contracting:

  • ๐Ÿ“ Collaborative Planning: Develop a care plan in partnership with clients, incorporating their needs and priorities.

  • โณ Progress Monitoring: Regularly evaluate progress and renegotiate goals for growth.

  • ๐Ÿ“† Ensure Follow-Ups: Follow up on missed or canceled services.

  • ๐Ÿ”„ Ongoing Reassessment: Re-negotiate planning at regular intervals to address evolving client needs.

  • ๐ŸŒŸ Clinical Excellence:

  • ๐ŸŒฆ Effective Intervention: Utilize appropriate interventions, both in non-crisis and crisis situations.

  • โค Empathy and Understanding: Create a judgment-free, empathetic space for clients, respecting their feelings and personal presentation.

  • ๐ŸŽฏ Tailored Approaches: Adapt your approach to meet client needs, whether it's individual, family, group, or crisis intervention.

  • ๐Ÿ’Š Medication Management: Consult with psychiatrists, monitor medication regimens, and attend to medication appointments.

  • ๐Ÿคน Case Management: Provide case management services as needed.

  • ๐Ÿค Collaborative Care: Advocate, coordinate, and maintain contact with other providers involved in clients' care.

  • ๐ŸŒŸ Trauma-Informed Care: Incorporate trauma-informed care models.

  • ๐Ÿšช Discharge and Termination:

  • ๐Ÿ Goal Achievement: Identify when therapeutic goals have been met and work toward appropriate discharge.

  • ๐Ÿ—ฃ Client Preparation: Discuss progress with clients and facilitate timely discharge planning.

  • ๐Ÿ”€ Transfer Assistance: Facilitate transfers/referrals for further services when appropriate.

  • ๐Ÿ“ Supervisory Consultation: Consult with clinical and program supervisors before discharge.

  • ๐Ÿ“œ Paperwork and Planning: Complete termination paperwork and discharge planning promptly.

  • ๐Ÿ“ Documentation, Meetings, and Productivity:

  • ๐Ÿ“„ Accurate Documentation: Complete documentation following agency and program guidelines.

  • ๐Ÿค Collaborative Meetings: Facilitate meetings and collaborate with referral sources as needed.

  • ๐Ÿ—“๏ธ Attend Team Meetings: Actively participate in internal team meetings and case presentations.

  • ๐Ÿš€ Meet or Exceed Productivity Targets: Maintain or exceed program productivity requirements.

๐Ÿš— Transportation Responsibilities:

  • ๐Ÿš— Vehicle Safety: Maintain a professional and safe environment in agency vehicles.
  • ๐Ÿ›ก๏ธ Child Safety: Ensure the safety of children and adults during transportation.
  • ๐Ÿงฝ Vehicle Maintenance: Monitor and document vehicle maintenance regularly.
  • ๐Ÿ—‚๏ธ Log Maintenance: Keep required vehicle logs and maintain records.
  • ๐Ÿšฆ Transportation Coordination: Prepare for pick-up and drop-off of clients for program services.
  • ๐Ÿ—ฃ๏ธ Clear Communication: Communicate effectively with parents, caregivers, and children during transportation.

๐Ÿ“œ Qualifications:

To excel in this role, you should possess:

  • ๐Ÿš— Valid NJ State Driver's License in Good Standing.
  • ๐Ÿ“š Master's Degree in Social Work or equivalent. LSW, LPC, LMFT, or state licensure specific to your degree.
  • ๐Ÿ‘ฅ Two years' experience working with children and families, especially in child welfare.
  • ๐Ÿ’ช Experience with diverse populations in an urban setting.
  • ๐Ÿง  Knowledge of mental health, various treatment modalities, and community resources.
  • ๐Ÿšจ Crisis Intervention Skills: Ability to apply appropriate interventions in crisis and non-crisis situations.
  • ๐ŸŒˆ Trauma-Informed Care Knowledge: Familiarity with interventions for families experiencing abuse or trauma.
  • ๐Ÿข Organizational Skills: Solid organizational and time management skills.
  • ๐ŸŒŸ Team Player: Ability to work well both independently and as part of a team.
  • ๐Ÿ—บ Navigation Skills: Ability to support clients in navigating multiple care systems.
