Position summary
Resident Care Coordinator provides direct personal care and supervision to the clients at the facility. The staffing pattern for this department is primary care. Promotes resident wellbeing and satisfaction through support with activities of daily living. Communicates with other facility departments to ensure resident needs are met.
Tasks / Responsibilities (To include, but not limited to):
Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of residents as needed.
Assist the Administrator in coordinating incidental medical, dental, vision, hearing, and podiatry care for residents and arrange resident escorts as needed.
Participate in the community’s resident assessment program in accordance with company policy and state regulations.
Recruit, develop, manage and schedule Care Providers and Medication Technicians.
Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
Encourage teamwork through cooperative interactions with co-workers and other departments.
Audit patients’ charts, MARS as needed
Assist with activities of daily living including assisting with resident self-administration of medications assigned, following facility protocol, licensing regulations and guidelines for both resident and employee safety.
Follow safety guidelines in the facility, including proper lifting techniques and universal precautions when providing care to the residents. Report any injuries to the administrator immediately.
Follow the schedule of duties for the caregiver as well as the individual plan of care for each resident.
Function as a team assisting co-workers as the need arises.
Monitor resident activity, food intake, functional status, psychosocial status, and taking action as required to promote resident well-being.
Report resident status change and incidents immediately to the supervisor.
Act immediately on any resident crisis following protocol and basic first aid training.
Document resident status change including but not limited to physical change, reaction to medication, psychosocial status change.
In the event all assigned duties cannot be completed ask for assistance and report to the administrator.
Serve as role model.
Provides crisis intervention services when authorized and necessary.
Maintain professional standards and follows the policies and procedures of the organization to include all federal, state, and local regulations.
Maintain current cardio-pulmonary resuscitation (CPR)/ first aid (FA) certifications and crisis prevention and intervention (CPI) annual training.
Promote open communications between healthcare professionals, families, residents, and staff.
Adhere to guidelines in the employee handbook including dress code, conduct, scheduling etc
Complete Incident Reports
Any other assignments made by the administrator

Environmental/Physical Requirements

Regularly required: to stand, walk, stoop, kneel or crouch and lift and/or move clients weighing approximately 180 pounds; reach with hands and arms, and talk or hear.
Preferred Experience/ Minimum Qualifications
Must be at least eighteen (18) years of age.
High school diploma or equivalent.
Medical Technician Certification preferred
Prefer one (1) year of experience supervising and managing employees.
Ability to effectively train staff.
Valid Basic Life Support (BLS)/ Cardio-Pulmonary Resuscitation (CPR)/ First Aid (FA) certification.
Elder care experience is preferred.
Flexibility in work schedule as evidenced by working holidays, evenings, weekends, and additional shifts when necessary to ensure adequate coverage within the department.
Able to operate a personal computer and be proficient in MS Office programs (Word, Excel, Publisher, PowerPoint, etc.).
