We are searching for an VOCA Case Manager to join our team:

VOCA Case Management Program (CM) is funded through the Office of Victim Services to address an identified need of case management services for victims of crime. VOCA CM recognizes the broad impact that an act of crime has on a family’s life and well-being. VOCA CM provides holistic case management services to promote safety, self-sufficiency and resiliency for persons in CT who have suffered, directly or indirectly, a physical, emotional, or personal loss as a result of a criminal act. VOCA CM will promote equitable access to services and a continuum of care through coordination with CCFSA partner agencies and other community agencies. VOCA CM program offers home-based or community-based visits depending on the preference and need of the client.

The Case Manager (CM) provides outreach, initial assessment, advocacy, case planning, service coordination, and case management for clients referred to the VOCA CM program. Also conducts targeted outreach to locate assigned VOCA CM referred families within designated program time frames.

  • Engages located families in VOCA CM services in alignment with expected program outcome standards
  • Provides intensive, client-centered, in-home and/or community-based case management
  • Performs initial client assessments and updated assessments within designated program time frames
  • Works with clients to develop and implement clear and relevant goals designed to assist clients in overcoming barriers to higher levels of functioning and well-being
  • Advocates for clients to help them navigate the legal systems and access Victim Fund

Requirements:

  • BSW and/or equivalent Human Service degree with at least 4 years’ experience working in the human service field.
  • Knowledge of VOCA services and systems in CT
  • Significant knowledge of regional community-based services for victims of crime
  • Demonstrated ability to work with culturally and linguistically diverse families
  • Demonstrated ability in client-centered, home-based case management services
  • Demonstrated ability in creating and implementing individualized service plans
  • Ability to work well independently
  • Ability to maintain a flexible schedule, to include non-traditional hours (nights/weekends)
  • Dedicated to promoting cultural wellness on the job
  • Demonstrated basic computer literacy, ability to work in a Windows environment, and utilization of mobile devices
  • Demonstrated ability to work in a team approach
  • Must have a valid driver's license and reliable transportation to commute to visits and work
