Fall River Jewish Home, a 62 bed skilled nursing facility in Fall River, MA is seeking a qualified, experienced MDS Coordinator (LVN or RN) for full-time employment. Ideal candidates will have at least 3 yrs of recent MDS experience within a SNF since the inception of PDPM.

The primary purpose of your position is to conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment.

Responsibilities:
- Conduct the resident assessment instrument for all assigned residents, within accordance of rules, regulations, and guidelines
- Ensure accurate and timely completion of MDS (Minimum Data Set) assessments
- Collaborate with interdisciplinary team members to gather necessary information for assessments, and to identify any barriers to success impacting quality measures

- Provide education and training to staff on proper documentation practices

-Analyze and report on key performance indicators to measure success and proactively identify areas of need

-Maintain and share expertise/knowledge related to reimbursement and quality outcomes

-Work in conjunction with teams to resolve issues, endorse changes and conduct follow-ups to establish that recommendations are effectively implemented and monitored for appropriateness

-Promote compliance by performing periodic audits of assessments, supporting documentation, and other relevant data

Skills:
-Must be an RN or LVN with MA licensure, RAC-CT preferred

- Proficiency in utilizing electronic health record systems for documentation purposes
- Knowledge of MDS assessment process and requirements
- Excellent attention to detail and ability to identify errors or inconsistencies in medical records
- Strong communication skills to effectively collaborate with interdisciplinary team members.
