Job Description:

• Available on phone during on-call hours
• Triage and asses patients condition
• Home visits after hours
• Can provide quality care for patients and family
• Communicate with staff members including: Care Coordinators, Registered Nurses, Medical Directors, Intake Coordinators...
• Completes reports and documentation with quality and in a timely matter.

Job Types: Full-time, Part-time

Pay: $30.00 - $45.00 per hour

Standard shift:

• Day shift
• Evening shift
• Night shift
• Overnight shift

Supplemental schedule:

• On call

Weekly schedule:

• Monday to Friday
• Rotating weekends
• Weekends as needed
