Job Overview: The Wound Care Director plays a crucial role in a skilled rehab nursing home by overseeing and coordinating all aspects of wound care for residents. Responsibilities:
Provide expert wound care assessment, treatment, and management to residents with acute and chronic wounds.
Develop, implement, and evaluate individualized wound care plans based on thorough assessments and collaboration with the healthcare team.
Conduct regular wound rounds to assess resident wounds, monitor progress, and provide appropriate interventions.
Utilize evidence-based practices to promote optimal wound healing and prevent complications.
Educate nursing staff, residents, and families about wound care techniques, prevention strategies, and appropriate documentation.
Stay updated with the latest advancements in wound care treatments, products, and technologies.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate wound care interventions and ensure consistent care delivery.
Monitor and document wound healing progress, treatment modalities, and resident response to interventions accurately and in a timely manner.
Implement infection control measures and adhere to safety protocols to prevent wound-related infections.
Participate in quality improvement initiatives to enhance wound care outcomes and resident satisfaction.
Maintain accurate and comprehensive wound care records, including assessment findings, treatment plans, progress notes, and discharge summaries.
Conduct staff training and in-services related to wound care best practices, prevention, and management.
Ensure compliance with regulatory standards, policies, and procedures related to wound care.
Collaborate with external wound care specialists, consultants, and vendors to enhance resident care and treatment options.
Provide support and guidance to nursing staff in wound care documentation, assessment techniques, and treatment modalities.
Qualifications:
Current registered nurse (RN) license in good standing.
Bachelor's degree in nursing (BSN) preferred.
Certification in wound care management (e.g., Wound Care Certification Board) highly desirable.
Minimum of 2 years of experience in wound care management, preferably in a skilled rehab nursing home setting.
Extensive knowledge of wound assessment, classification, and treatment modalities, including wound vacuums, dressings, and advanced wound care products.
Proficient in utilizing electronic medical records (EMR) systems and other relevant software applications.
Strong clinical judgment and critical thinking skills to develop effective wound care plans and interventions.
Excellent communication and interpersonal skills to collaborate with the interdisciplinary team, residents, and families.
Ability to educate and train nursing staff on wound care best practices and protocols.
Detail-oriented with exceptional organizational skills to maintain accurate documentation and track wound care outcomes.
Knowledge of regulatory requirements and guidelines related to wound care and infection control.
Ability to work independently and prioritize tasks effectively in a fast-paced environment.
Demonstrated leadership qualities and the ability to supervise and mentor nursing staff in wound care practices.
