Job Title: Registered Nurse (RN) - Skilled Rehab Nursing Home
Job Summary: The Registered Nurse (RN) working in a skilled rehab nursing home/hospital is responsible for providing comprehensive nursing care to residents in a rehabilitative and long-term care setting. The RN will collaborate with the interdisciplinary healthcare team to assess, plan, implement, and evaluate individualized care plans. The primary focus of this role is to promote and maintain the health and well-being of residents, facilitate their recovery, and support their transition back to independent living whenever possible.
Duties and Responsibilities:
Resident Care:Conduct thorough assessments of residents' physical, psychological, and social needs.
Develop, implement, and update individualized care plans based on the assessment findings.
Administer medications, treatments, and nursing interventions as prescribed.
Monitor residents' progress, document observations, and adjust care plans accordingly.
Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility.
Provide emotional support and education to residents and their families regarding their conditions, treatments, and care plans.
Collaborative Approach:Collaborate with a multidisciplinary team, including physicians, therapists, social workers, and nutritionists, to coordinate comprehensive care for residents.
Participate in care conferences and contribute to developing and revising care plans.
Communicate effectively with team members to ensure continuity of care and promote positive outcomes.
Coordinate and supervise the work of nursing assistants and other healthcare staff to ensure quality care delivery.
Rehabilitation Support:Facilitate and monitor residents' participation in rehabilitative therapies, such as physical therapy, occupational therapy, and speech therapy.
Assist in planning and implementing therapeutic activities to enhance residents' functional abilities.
Evaluate residents' progress in rehabilitation and modify care plans accordingly.
Educate residents and their families about exercises, mobility aids, and adaptive equipment to maximize independence.
Safety and Compliance:Follow established nursing standards, policies, and procedures to provide safe and effective care.
Adhere to infection control protocols and maintain a clean and safe environment.
Ensure accurate and timely documentation of residents' medical records, assessments, and interventions.
Comply with regulatory guidelines and participate in quality improvement initiatives.
Professional Development:Stay updated on current nursing practices, trends, and evidence-based interventions related to rehabilitation and long-term care.
Attend educational programs, workshops, and seminars to enhance clinical knowledge and skills.
Engage in self-assessment and identify areas for professional growth and development.
Foster a culture of continuous learning and share knowledge with colleagues.
