Seeking Dynamic Pediatric Speech Language Pathologist to join the M Health Fairview Rehabilitation Services team! This position will deliver safe and effective patient care within scope of practice to primarily a pediatric population.
M Health Fairview Rehabilitation offers a broad range of services that serve patients across 10 acute-care hospitals, 3 post-acute settings and 70 outpatient adult and pediatric therapy clinics. Consisting of Physical, Occupational and Speech Therapy as well as Audiology and Cardiac & Pulmonary Rehab, our therapists collaborate with colleagues in all medical settings and offer dozens of specialty programs.

Rehab’s continuing education program offers 40 continuing education courses per year at no cost to employees and further supports numerous external CE courses each year.

Responsibilities Job DescriptionThe Speech Language Pathologist partners with the patient and family while performing the Evaluation/Re-evaluation. Captures and applies pertinent data, performs examination, and identifies treatment diagnosis
Develops an individualized care plan through coordination and collaboration
Analyzes data to identify needs and potential risks, settles prognosis for recovery, and establishes individualized goals.
Implements and modifies care plan based on reassessment and patient response
Coordinates care and communicates effectively with interdisciplinary team for all aspects care & establishes appropriate discharge plan
Partners with patient and care team to provide skilled intervention
Implements and alters treatment to reflect change in patient status and response to treatment
Assesses patient and/or caregiver learning needs and provides appropriate education
Documentation/Billing
Uses acceptable medical terminology and follows professional practice standards.
Completes documentation in a timely manner per department standardsStays in sync with department billing standards
Professional Expectations
Crafting and implementing improvements, and participates in and supports entity/system initiatives. Adaptable in responding to demands of multiple processes and supervisors.
QualificationsRequired
Certification by ASHA and Speech Therapy Practitioner license in the state of Minnesota, or eligible for Temporary Minnesota License per Clinical Fellowship acceptance.Basic Life Support certification
