POSITION: Intake and Diversion Coordinator

RESPONSIBLE TO: Program Director-Family Housing Stabilization

CATEGORY: EXEMPT

FACETS is looking for a mission-driven Intake and Diversion Coordinator to join our team serving families experiencing homelessness or at-risk of homelessness in Region 1 of Fairfax County. The Intake and Diversion Coordinator triages referrals for the Family Homeless Services System in Region 1 of Fairfax County including: conducting emergency shelter intake and orientation for households with children referred to the Next Steps Family Program located in scattered site apartments in Alexandria, VA, providing shelter diversion services to households, and assigning prevention/diversion cases to Community Case Managers.

The successful candidate will have experience and knowledge of homeless services, with familiarity with Family Homeless Services System in Fairfax County a big plus. If you’re well-organized, an empathetic problem solver, a fast learner, highly motivated, skilled in providing services in the community, and bring excellent data entry and organizational skills come join our team. Bachelor’s degree required. Bi-lingual (English/Spanish) preferred.

RESPONSIBILITIES:

  • Receives and coordinates all emergency shelter and prevention/diversion referrals for the Family Homeless Services System in Region 1 of Fairfax County from Coordinated Service Planning (CSP) and other appropriate agencies within the Fairfax-Falls Church Community Partnership to Prevent and End Homelessness.
  • Responsible for the daily review of family homeless services referrals containing detailed confidential information on households experiencing homelessness or at-risk of homelessness.
  • Assesses housing barriers of households with children who are homeless or at-risk of homelessness to determine housing and service needs.
  • Triages households referred to the Next Steps Family Program to explore other stable housing options in an effort to divert them from emergency shelter.
  • Coordinates the intake process for households with children referred to Next Steps Family Program by CSP and serves as the primary contact.
  • Ensures households understand rules, rights, and responsibilities upon entry into the shelter program. Assists with shelter orientation.
  • Ensures that the established shelter triage and entry process is followed properly and that new households are met with as quickly as possible.
  • Coordinates with the Program Director-Family Housing Stabilization and the Facilities Specialist on the status of vacancies and unit conditions.
  • Helps maintain updated household documentation, emergency information, and assist with resource referrals and tracking household progress within shelter system.
  • Uses automated technology including HMIS and hard copy files to maintain, update, and report on case data, goal attainment, and outcomes in a timely manner.
  • Attends various regional meetings including Intake Coordinator Meetings and Continuum of Care meetings throughout Fairfax County, and generates and distributes housing resources to FACETS staff.
  • Ability to work a flexible schedule including nights and weekends.
  • Represents FACETS in the community, in accord with Agency Mission, Vision, and Code of Ethics.
  • Performs other duties as assigned.

QUALIFICATIONS:

· Bachelor’s Degree in a human services field.

· Two years of professional social work/case management experience with homeless and/or at-risk populations or experience working in an emergency shelter setting

· Bilingual (English/Spanish) a preferred;

· Master’s Degree a plus

  • Ability to work with households experiencing crisis, assess complex situations, and develop appropriate approaches (shelter intake, diversion, prevention referral)
  • Knowledge of current social service, homeless, and housing issues and methods/approaches to address issues.
  • Ability to work independently in a fast-paced environment and manage tasks and workload with minimal supervision.

· Excellent data entry and data quality skills; ability to use HMIS to establish and maintain case records and to facilitate data collection.

· Ability to communicate clearly and concisely, both orally and in writing.

· Ability to work a flexible schedule

· Must pass criminal background and Child Protective Services background checks.
