Position OverviewUnder the direct supervision of the Clinical Program Manager, the Case Manager is responsible for providing case management services to participants and their families with appropriate referrals and resources to assist them in sustainability. Case Manager will support services to assist participants with independent living.DutiesContact new referrals within seventy-two hours of receipt of case.Maintain a maximum caseload of twenty participants.Provide in-home supportive services for participants and their families in accordance with program/agency standards.Conduct intake assessment and develop participant service plan.Provide two home visits monthly (additional as needed).Maintain caseload documentation in compliance with all agency policies and procedures as well as federal, state, county and local regulations. Submit documentation to Program Supervisor for review.Provide case management services, including but not limited to linking participants to auxiliary services and assisting participants and their families with accessing appropriate resources.Assess and coordinate service provision to address identified social, emotional, financial and medical needs of participant and family.Coordinate and maintain appropriate service provision with referral source, i.e. DCFS and Department of Probation.Maintain positive working relationship with local linkage and referral agencies.Maintain a working knowledge of existing community programs, the services provided and eligibility requirements.Attend all staff meetings, case conference, in-service, community trainings/meetings as required/requested.Assist in the dissemination of materials to promote services and programs to the community.Develops and maintains interventionist skills through seminars, workshops, webinars, or other forms of training/education related to working with adults.Develop and maintain a reference list of services provided by private and public agencies.Performs other duties as requested, directed or assigned.Performs other duties as directed by Supervisor.Must have ability to nurture respectful relationship with clients, children and must be sensitive to the cultural and socioeconomic characteristics.Qualifications and Experience Requirements Strong working knowledge of case management, documentation, and multicultural and socio-economic issues.Know and comply with Agency policies and procedures, HIPAA, Department of Mental Health, policies and documentation guidelines, and other state/federal regulations relating to service programs for at risk populations.Basic advocacy skills.Present ideas, information, and viewpoints clearly, both verbally and in writing.Efficiently use the personal computer to include a variety or word processing software.Utilize analysis, experience, and judgment to make decisions.Adapt to changing needs by acquiring new skills and knowledge.Provides services within a culturally sensitive, ethnic informed model of practice and is respectful to the community and participants.Good communication and interpersonal skills.Bilingual (E/S) required.Education and Certification Requirements A Bachelor’s degree in Social Work or related field.At least two (2) years’ experience working with low-income children and families.Other Requirements Must have reliable automobile for use on the job (mileage to be reimbursed). Valid Class C driver’s license, transportation and State required automobile insurance coverage.T.B clearance, to be renewed every two years.Physical Requirements To perform this job the individual must be able to carry out all essential functions satisfactorily. Approximately 50% of time is spent sitting, while frequently required to walk, stand and bend. Must be able to speak and hear well. Good vision is imperative. Occasionally required to stoop, kneel, crouch or crawl.
