Extended Home Care is looking for an experienced and reliable Intake Specialist to fill an open position in our Brooklyn office. As an Intake Specialist, you will be responsible for reviewing and processing all service referrals. You will be responsible of gathering all patient information, ensuring all data provided on the referral is complete, and accurate, as well as additional duties and responsibilities within the scope of the position.
Responsibilities:

• Responsible to review and correct new patient profile coming from all different sources.
• Responsible to review and correct physician information on each case.
• Responsible to collect and track any missing or incorrect physician information.
• Responsible to collect from Home Care Reports, patients information for potential Transitioning from Part A to Med-B, add them as new patient to the MED B tracker.
• Home care report MUST be checked at least once a day.
• Responsible to add all patient information received on referral as well as responsible to obtain any necessary missing information to complete patients demographics and insurance information
• Ensure insurance information is added and verified, primary and secondary if applies as well as Co pays.
• Responsible to verify potential Discharge date
• Responsible to manage attachments on Med-B tracker of all documents needed for the Intake process, and ensure information matches patient profile
• Responsible to add all relevant referral information as notes in the Med-B tracker
• Ensure Intake Status is updated and follow up date added
• Ensure referral is assigned to an Intake Coordinator for future follow ups
