POSITION SUMMARY:The Wound Care nurse will deliver quality wound care and preventative measures to maintain skin integrity to assigned residents in compliance with applicable federal and state guidelines and established policies and procedures. The Wound Care nurse will work under the direction of the Director of Nursing or assigned designee.

ESSENTIAL JOB FUNCTIONS:

• Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of residents wound and skin integrity.
• Collecting and documentation of residents’ skin status including medical history.
• Wound care prevention and prevention through plan of care. Educates staff on the importance of quality care to prevent skin breakdown.
• Sets priorities for nursing action for wound care and prevention.
• Communicates with physicians regarding resident wound needs.
• Initiates nutritional interventions to ensure that residents with wounds are provided nutrition to promote wound healing.
• Performs direct resident care utilizing established procedure, policies, and standards.
• Administer treatments and procedures in accordance with the physician’s orders.
• Maintains the comfort, privacy, and dignity of each resident while performing treatments.
• Communicates and interacts effectively, courteously, and tactfully with residents, visitors, family members, peers, and supervisors.
• Conducts rounds frequently to ensure that residents with incontinence are receiving peri care and preventative ointments timely.
• Performs duties in a safe manner in accordance with established policies and procedures.
• Performs all job responsibilities in accordance with prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves and proper disposal of soiled material.

Candidates must possess:

·Current Illinois LPN License

·Two (2) years previous experience in nursing preferred with preference given to Long term Care Experience.

-Previous wound care experience.

·Current CPR certification.
