Activities (most critical to least critical)
1. Complete in-home speech therapy evaluations and weekly therapy visits.
2. Develop and implement therapy plans focusing on improving/restoring speech and language functioning.
3. Complete thorough and timely documentation of visits and services provided to recipients and families.
4. Instruct and familiarize the recipient and pertinent family members with all aspects of the total treatment plan. Complete Family Training, as appropriate, with family members to ensure maximum benefit and continuation of treatment.
5. Conduct initial evaluations of new recipients and re-evaluations of current recipients.
6. Ensure federal and state compliance to maintain licensure. Complete required CEUs.
7. Attend meetings with recipients/families, CMH Social Workers, and others, as appropriate.
8. Maintain communication with therapy services office staff.
9. Periodically report to the physician on the recipient’s treatment, response, and progress, and assist the physician in determining any necessary changes in the treatment plan.
10. Participate in staff meetings, case conferences and trainings as required.
