Exodus Recovery, Inc is looking to hire a full-time Health and Prevention Care Manager (LVN) for our new program located in Culver City, CA.

POSITION SUMMARY:

Exodus Recovery, Inc. (Exodus) shall employ a Licensed Vocational Nurse as a Health and Prevention Care Manager to provide individualized and integrated services focusing on prevention and health promotion of physical and mental wellness.

MINIMUM QUALIFICATIONS:

Licensed as a Vocational Nurse and one (1) year clinical experience in a health-related field. Strong verbal and written communication skills. Strong knowledge of medical terminology and experience in evaluation, direct client service and community linkage. Ability to work independently and as a member of a multidisciplinary team, and show mature judgment. Ability to collaborate with staff from other disciplines and with community agencies.

EDUCATION:

• Licensed as a Vocational Nurse.
• One (1) year of experience in providing direct client services.
• Current CPR Certification required and current CPI Certification preferred.

Commensurate with experience.

Job Type: Full-time

Pay: $27.00 - $30.00 per hour
