Job Title: MDS Assistant

Shift: 7:30 am to 4:00 pm

Pay Range: $35 - $37 per hour

Are you a skilled MDS Assistant looking for a rewarding opportunity? Join our team at Charlotte Bay Rehab and Care Center! We are currently seeking a dedicated individual to support our MDS Coordinator in conducting resident assessments and ensuring compliance with regulatory standards.

Duties and Responsibilities:

• Assist the MDS Coordinator in conducting resident assessments (MDS) in accordance with regulations.
• Help maintain and update policies and procedures related to resident assessments and care plans.
• Perform administrative tasks such as completing medical forms and reports.
• Participate in facility surveys conducted by government agencies.
• Monitor Quality Improvement (QI) and Quality Measures (QM) reports and assist in implementing corrective actions.
• Ensure timely completion and transmission of assessments, reporting any issues to the Administrator.
• Collaborate with the Interdisciplinary Care Plan Team to develop comprehensive care plans for residents.
• Foster positive relationships with healthcare professionals and government agencies involved in resident assessment and care planning.
• Schedule resident assessment and care plan meetings.
• Issue Advance Beneficiary Notices (ABNs) to Medicare Part A residents.

Specific Requirements:

• Proficiency in English language skills.
• Ability to make independent decisions.
• Licensed Nurse with prior MDS experience preferred.
