Overview:

Evolve Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. As an LPN, you will play a vital role in providing quality care to our patients. This position offers an opportunity to work in various healthcare settings, including home care, assisted living facilities, and hospitals.

Duties:

• Administer medications and treatments as prescribed by physicians
• Monitor and record vital signs of patients
• Assist with activities of daily living, such as bathing, dressing, and feeding
• Perform catheterization and tube feeding procedures
• Collaborate with the healthcare team to develop and implement patient care plans
• Maintain accurate and up-to-date medical records using ICD-10 coding system
• Provide emotional support and education to patients and their families
• Assist in emergency situations, such as in the ER or during medical emergencies
• Lift and transfer patients safely using proper body mechanics

Qualifications:

• Valid LPN license in [state]
• Proven experience in a healthcare setting, preferably in home care or assisted living
• Proficient in performing catheterization, tube feeding, and vital sign monitoring
• Knowledge of ICD-10 coding system for accurate medical record documentation
• Strong communication and interpersonal skills to effectively interact with patients, Families, and healthcare professionals
• Ability to work independently and as part of a team in a fast-paced environment
• - Familiarity with ophthalmology procedures is a plus
