Position Summary

This role empowers and partners with youth who are within, and in transition out of, the Child Welfare System. This position will: support youth in rural Minnesota through their transition to adulthood, develop plans alongside youth for permanency or independence, and research / document information about youth’s social and medical history. This position is a great fit for someone who is communicative and organized, wants to make the lives of youth better, and enjoys driving across Minnesota.

Essential Functions

1. Strong ability to develop positive and trusting relationships with youth.
2. Utilize, teach, and model age-appropriate interventions with youth.
3. Gather information on youth’s personal history, relationships, experiences, traditions, health, trauma experiences, support network, and strengths to complete comprehensive youth assessments.
4. In collaboration with youth, develop an Independent Living Skills plan.
5. Prepare group curriculum and facilitate 10-week group sessions with youth.
6. Make recommendations for placement of children based on assessment of family capacity, skills, and abilities.
7. Facilitate discussion and education around identity, transracial complexities, and trauma-informed care.
8. Complete outreach to counties, community agencies, and youth to build partnerships and increase referrals.
9. Facilitate home visits with each client 1-3 times/month to review progress and goal development.
10. Complete required documentation within established deadlines. Arrive to meetings on time. Communicate with supervisor about any issues in meeting caseload requirements.
11. Every 6 months, set 2 personal goals for growth in Diversity Equity Inclusion work, and share progress.
12. Participate in monthly discussions or learnings about Diversity Equity Inclusion topics at team meetings.

Licensure / Credentials

This person must be able to pass a background check and FBI clearance check.
This person must have a valid driver’s license and a reliable vehicle.
It is preferred this person has familiarity working with youth and the child welfare system.

Work Environment

This hybrid role is for a Minnesota or Wisconsin based employee who can reliably commute to Oak Park Heights, MN, and across greater Minnesota as a position requirement. The role requires driving for up to four hours at a time. The EVOLVE workspace is a shared office space in close proximity to other department peers. The EVOLVE office is a pet-friendly workplace. All employees are provided with a laptop to work from home, provided the employee maintains high-speed internet and cell phone service. Your home workspace must be conducive to performing your role. Whether at home, or at the office, this role requires sitting for extended periods of time at a desk, while utilizing a computer to complete work. The hours of this role typically fall between 8:30a-4:30p with some afterhours required to meet families when they need. EVOLVE has opportunities to work at weekend events and programs.
