This important role helps ETS accomplish our mission! The TIM SUDP(T) shall serve as the primary care team coordinator for their assigned patients. Through community outreach and engagement, the SUDP(T) will connect individuals with Evergreen Services Mobile Dispensary (TIM) and other community and clinic-based services. The SUDP(T), in accordance with agency policy and procedures, federal, state, and local guidelines, shall provide assigned patients with, assessment, individual and group counseling, case management, community-based outreach and engagement, and coordinate overall patient care while enrolled at Evergreen Treatment Services.
