Evergreen Health & Rehabilitation is looking for a Certified Occupational Therapy Assistant PRN to join our team!

1. • Implements and executes an established patient treatment plan under the supervision of a licensed Occupational Therapist.
2. • Communicates with patient, families, referring physicians, and other members of the team.
3. • Provides timely written documentation per facility and department requirements.
4. • Develops and enhances clinical and professional skills through knowledge and professional association.
5. • Provides timely written documentation as required by insurance provider to meet licensing, certification, regulatory, reimbursement requirements.
6. • Supports and participates in department operations and development.
7. • Reports any changes in patient status or needs to supervising therapist on a timely basis.
8. ASSOCIATED RESPONSIBILITIES:
9. • Prepares and submits patient charges accurately to provide appropriate billing information.
10. • Meets or exceeds the productivity standards as established with Supervisor.
11. • Uses professional judgment to ensure safety of self, patients, and others at all times.
12. • Achieves quality patient care through interdisciplinary communication.
13. • Attends required meetings as designed by the Director of Rehab and/or Supervisor.
14. • Effectively communicates with therapists concerning patients' response to treatment plan.
15. • Provides constructive feedback to rehab aides.
16. • Attends in-services, training sessions or other educational presentations.
17.  • Coordinates treatment goals with other team members.
18. • Maintains valid licensure, certification or equivalent as requested by appropriate state agency.
19. • Maintains awareness of patient and community needs, conditions, and programs.
20. Other Duties as assigned.
21. PHYSICAL REQUIREMENTS:
22. • Must be able to move intermittently throughout the workday.
23. • Must be able to lift over 100 pounds with assistance according to the resident's abilities.
24. • Must be able to sit and walk for extended periods of time.
25. • Must be able to travel to reach assigned destination in a reasonable time frame.

Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience and may fall outside of the range shown.
