Description

SUMMARY:
The LVN/LPN, with appropriate competency completions, is responsible for direct care of patients ranging in age from 18 years to geriatric experiencing rehabilitation diagnosis. Demonstrates knowledge to carry out the patient’s plan of care. Satisfies the established standards of the nursing practice. Observes patient behavior, vital signs, symptoms, responses to treatment and reports changes to the Registered Nurse. Promotes teamwork with physicians and throughout all departments.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
May include, but is not limited to the following:
Job Specific:

  • Administers personal and therapeutic measures to patients as directed by physicians and/or nurses.
  • Prepares and administers medications as prescribed by the physician. May start and administer intravenous fluids and medications after successful completion of approved IV course and skills verification except for intravenous push medications.
  • Collects information and data that can assist the Registered Nurse in formulating a patient care plan.
  • Inspects equipment and facilities visually and physically to ensure patient safety and proper functioning.
  • Maintains current customer service, clinical, and technical skills by attending training programs and seminars to continue level of competency and ensure compliance with departmental policies and procedures. Attends and participates in meetings related to clinical activities.
  • Demonstrates ability to work cohesively and professionally in an interdisciplinary model with therapy, case management, physician's, laboratory, radiology, pharmacy, quality, food service/dietary, leadership, etc.
  • Incorporates factors related to safety into delivery of patient care as directed by the physician.
  • Demonstrates respect, courtesy, understanding and tolerance while accomplishing shared goals.
  • Meets learning needs of patients and significant others by providing accurate information regarding care during hospitalization including expected outcomes, home care instructions and referrals to appropriate health agencies and resources.
  • Responsible for establishing and maintaining learning environment by working effectively with health team members, assisting with care and documentation and participating in patient care team conferences.
  • Documents medication, administration, functional care, intake and output, vital signs and other patient related measures.

Job Function:

  • Provides nursing care for assigned patients by maintaining a current care plan and adjusting it to the patient's changing needs in conjunction with the Registered Nurse.

Education

  • Graduate from a school of vocational/practical nursing.

License/Certification

  • Currently licensed as an LVN/LPN in the state of practice. Current Basic Life Support (BLS) certification through the American Heart Association.

Experience

  • One (1) year experience in an inpatient healthcare or rehabilitation setting preferred. Working knowledge of personal computer and software applications used in job functions such as order entry, data input. Skilled in the usage of and able to learn other models of the Electronic Health Record/Electronic Medical Record (EHR/EMR) etc. Ability to project a professional image.

SUPERVISORY RESPONSIBILITIES:

  • Certified Nursing Assistants
  • Unit Secretary

OTHER REQUIREMENTS:

  • Participates in or conducts company-wide sponsored training to safeguard against improper use and disclosure of patient’s protected health information.
  • Monitors and ensures compliance with company policies, procedures, state and federal regulations and Accreditation Standards.

  • Adheres to all smoking or tobacco regulations established by corporate building owners, state or federal laws.

  • Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.
  • Reports to work on time as scheduled; adheres to policies regarding notification of absence.
  • Attends all mandatory in-services and staff meetings.
  • Represents the organization in a positive and professional manner.
  • Complies with all organizational policies regarding ethical business practices.
  • Communicates the mission, ethics, and goals of the hospital, as well as the focus statement of the department.
  • Maintains current licensure/certification for position, if applicable and supplies employer with copies of same per policy.
  • Consistently demonstrates superior customer service to patients, physicians, visitors, employees, and any other individuals with whom they may come in contact.
  • Consistently follows departmental and hospital Health, Safety, Security, Hazardous Materials policies and procedures.
  • HIPAA: Conduct job responsibilities in accordance with HIPAA privacy laws, follow hospital policy in provision of patient confidentiality. Able to identify patient confidentiality issues and reports to proper hospital personnel immediately.
  • Compliance: Conducts job responsibilities in accordance with standards set forth in Code of Conduct, policy and procedures, applicable federal and state laws, and applicable standards.
