ESSENTIAL DUTIES AND RESPONSIBILITIES: The following duties and responsibilities generally reflect the
expectations of this position but are not intended to be all inclusive, other duties may be assigned:
1. Territory assignments
Manage assignments of all providers based on patient and facility clinical need, census, geography, and other
factors
Assist with assigning providers to new buildings assigned to this care team
2. Patient registration
Discharge patients or change their registered services based on discharges, deaths, or changes to clinical
needs
3. Scheduling
Manage facility and patient level scheduling for all providers in each of the assigned patients/facilities
Ensure patients are scheduled for their routine visits as outlined in company best practices
Make adjustments as needed for PTO, sick days, or unusually high number of new referrals
Report reasons for missed visits or provider concerns to Care Team Clinical Manager for provider
management
4. Customer Service
Communicate with the facilities or patients ahead of scheduled visits
Ensure that facilities or patients receive copies of all scheduled patient visits
Receive calls from facilities and patients regarding any questions, issues, or concerns
5. Provider Support
Ensure that each patient is seen as medically necessary per clinical protocol. Scheduling rosters should be
updated daily to ensure accuracy.

Assist with any day-of changes to the scheduled visits due to acute needs, hospitalizations, admissions, or
discharges
Prepare clinical documentation for all the providers in the building each day
Complete basic opening of encounter notes
Perform medication reconciliation on all scheduled patients each encounter
Enter lab results and facility chart data on all scheduled patients each day into provider notes
Document appropriate time for CCM services
Track open notes for each provider and assist in closing of notes as able
6. Facility Support
Attendance is expected for days and hours scheduled
Share patient schedules with key facility staff as needed
Report any triage needs that can be communicated out to the appropriate provider
Assist with triage documentation as needed
7. Patient Support
Maintain patient confidentiality at all times
Check all forms of communication several times daily to ensure patient needs are met
Inform patient and family of recent visits, new orders and defer to provider when needed
Communicate transitions of care to CCM team to enhance patient care and education
8. Medical Assistant and Facility Medical Assistant Management
Be responsible for administrative and personnel management of assigned Medical Assistants
Evaluate and plan for Medical Assistants responsibilities
Assess and determine most appropriate assignments for Medical Assistants
Assist in recruiting, hiring and retention process of Medical Assistants
Assist new hire and ongoing training of Medical Assistants
9. Additional Duties and Responsibilities
Lead Medical Assistants are expected to be knowledgeable and adhere to Eventus WholeHealth standards,
policies and procedures and outlined in employee handbook including HIPAA rules and regulations
Lead Medical Assistant should adhere to the Eventus WholeHealth dress code policy i.e. clean, wrinkle-free,
matching scrubs with appropriate, closed-toed footwear and name badge when in a facility.
Lead Medical Assistant ensure office etiquette protocols are followed.
