Salary Range: $25.96 Hourly

Qualifications


Job Description -

a. Provide technical assistance, education, and outreach necessary to support the Care Coordinators and Care Managers. This includes, but is not limited to: community-based long term care subject knowledge, OCCO’s NJCHOICE and assessment for NF-LOC, and sensitivity to issues regarding social isolation, caregiving, physical disability, and cognitive disability.
b. Develop in-depth knowledge of the HCBS system, familiarity with community resources, and use of the Division's connectivity tools, such as Outlook, Zoom, Teams, etc.
c. Initiate, organize, and lead scheduled and other program meetings.
d. Consult, provide technical assistance, and where necessary make determinations regarding care management, care coordination, and other JACC program issues.
e. Act as a resource for peers, staff, County and State agencies, providers, and the community.
f. Draft policy memoranda, informational memoranda, program instruction, brochures, information, and fact sheets as necessary, and assist coworkers with their information development as assigned.
g. Maintain and actively share current knowledge and understanding of issues, models, research, services, and funding of long-term community care.
h. Assist team members as needed in the SAGE grants management system, including review and approval of contracts and payment vouchers.
i. Participate in training and other activities to learn the operation of other state and federally funded grants and programs as assigned.
j. Develop and provide on-site, telephone, written, webinar, etc., training to JACC network on program specifics, including program operation, contract applications, required and recommended tools, technology (i.e. HCBS, SAMS, or their successor(s)), and other program elements to ensure quality and support quality assurance goals.
k. Identify needed changes to currently drafted regulations and policies to keep programs consistent with the Division of Aging Services’ mission, goals, and practices. Communicate these as appropriate to the Division's Legal and Regulatory staff.
l. Initiate and draft updates to the JACC "Provider Manual" to comply with updated draft regulations and all other JACC-applicable policies and procedures. Work with appropriate colleagues’ workgroups, and committees to obtain feedback on the materials prior to final submission to the manager of the Office of AAA Administration for approval.
m. Provide feedback on manuals and other materials for related programs as assigned.
n. Conduct on-site and remote monitoring, evaluation, technical assistance, and training in support of the quality strategy and the Division’s mission and goals, and according to the established monitoring calendar. Observe and evaluate the contracted agency’s abilities in areas such as administration and organization, client population, staffing and supervision practices, and documentation. Conduct additional monitoring for at risk providers and in response to concerns or complaints.
o. Update monitoring tools and methods to support effective monitoring and to stay consistent with current monitoring trends.
p. Make written recommendations for provider compliance and plans of correction after monitoring activities and as necessary.
q. Make recommendations for overall program improvement.
r. Attend and/or facilitate in-house meetings for program and fiscal program and provider review.
s. Field and respond to inquiries and requests of all kinds (written, telephone, fax, email, etc.).
t. Field and respond to inquiries and requests from all sources (internal, external peers, colleagues, caregivers, providers, AAA/ADRCs and other governmental bodies, private organizations, and the general public).
