Duties:
Conducts a psychosocial assessment, evaluating patient/family psychosocial risk factors. Demonstrates problem solving and critical thinking through reassessment of the patient’s plan of care, adjusting goals and interventions as needed.
Evidenced by documentation in the medical record and/or team observation/feedback.
Based on clinical assessment, communicates recommendations with the patient, family and team to support a collaborative family centered approach. Ensures effective communication through patient handoffs with interdisciplinary team as evidenced by documentation in the medical record and/or team observation/feedback.
Develops an action plan with the patient, family and team taking into consideration variables that impact the patient’s plan of care to include social, emotional, physical or environmental factors as evidenced by documentation in the medical record and/or team observation/feedback.
Supports patient/family with resources related to end of life decision making. Provides a supportive presence with patient/families during end of life care as evidenced by documentation in the medical record and/or team observation/feedback. 5: Supports families with complex family dynamics, assessing for barriers to care and discharge.
Intervenes and de-escalates, as appropriate, to redirect non-productive behavior as evidenced by documentation in the medical record and/or team observation/feedback.
Provides support and intervention for families with legally complex cases evidenced by referral to appropriate community resources, or as documented in the medical record.
Assists in maintaining privacy and confidentiality of protected health information.

To assist patients and their families in making psychosocial adaptations specific to their diagnosis, while facilitating eligibility determination for social programs and funding resources. Apply optimal utilization of health care services and community resources in accordance with the principles of human growth and development. To provide clinical social work and therapeutic services to Clients patients and families utilizing a collaborative approach. The social worker will assess, plan, implement, deliver, monitor and evaluate the options and services required to address a patient's individualized plan of care in collaboration with the interdisciplinary team to achieve discharge goals
Education:
Master's Degree in Social Work LMSW - Licensed Master Social Worker or LCSW - Lic Clinical Social Worker or LMSW-AP - Lic Mas Soc Wkr - Adv Pract- Required
