Estates Healthcare and Rehabilitation Center is Hiring a MDS Coordinator Near Fort Worth, TX

We’re seeking an MDS Coordinator to join our growing team! Reliability, trustworthiness and consistency with regard to attendance is extremely important to this job. The ability to regularly and timely attend work, cooperative and politely work and deal with others, and to effectively multi-task and work in a stressful environment are also essential functions to this job.

Responsibilities:

  • Schedules residents for appropriate admission, annual, quarterly, Medicare assessment or change-of-condition assessments, overseeing communication to nursing and other departments.
  • Assures compliance with Federal and State regulations regarding completion/coordination of the comprehensive assessment (MDS).
  • Monitors MDS and care plan documentation on all residents, assuring that complete, orderly chronological and current documentation is in file.
  • Maintains proficiency in software.
  • Coordinate and attends regular Medicaid meetings to discuss current residents, pending residents and recipients.
  • Coordinate and attends all ITTM meetings in order to gather information, communicate changes, maintain and update the clinical records including the Care plan.
  • Performs care plan updates as needed.
  • Provide DON, Administrator and pertinent staff with the Quality Indicators Report.
  • Perform transmissions of the MDS on a timely basis.
  • Remains current with changes in the industry through attendance at outside workshops and conferences.
  • Perform Ancillary management, collection and reporting to corporate billing personnel.

Requirements:

  • Licensed Nurse in good standing.
  • Geriatric nursing background.
  • Restorative care philosophy, excellent professional skill (both assessment and hands-on and good communication and organization skills).
  • Working knowledge of nursing home regulations.
  • Ability to work independently as well as a meaningful member of the team.
  • Excellent organizational skills in order to coordinate many projects simultaneously.
