Job Description:
The Community Health Worker Supervisor oversees a team of Community Health Workers who support patients facing complex challenges to achieve their health goals, with a specific focus on behavioral health and the social factors that impact health outcomes. The supervisor provides support, coaching, and assists the Community Health Workers to integrate into clinic teams and community-based organizations.

• Recruits, trains, and leads a team of Community Health Workers
• Supports and coaches Community Health Workers
• Ensures practices and procedures are understood and followed
• Analyzes reports
• Supports Community Health Workers as they integrate within their clinic
• Collaborates with community-based organizations
• Supports program expansion to new clinic/community locations
• Participates in program evaluation

Pay Range
$52062 - $78104 / year

The approximate base compensation range is posted, but the actual offer will reflect our total compensation and benefits package. The offer will be determined by a number of factors including the selected candidate’s experience, knowledge, skills, abilities, FTE, as well as internal equity among our team.
