Position Title: Front Desk Receptionist
Job Summary: Front Desk staff will provide the highest quality care to patients by combining the latest advances in evidence-based medicine, patient-centered compassionate care, and leading-edge information technology, such as electronic medical records and e-prescriptions. Front Desk will provide outstanding customer service to potential Health Home Members. The Clerical Representatives will act as an intermediary between providers and members - serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher and assist with patient needs
Responsibilities include:
· Meet daily enrollment targets and care coordination services to members with chronic health conditions
· Conduct outreach activities specially to the loss to care patients
· Conduct community outreach visits to patients
· Provide expedited visit to patients for urgent situations such as hospitalization
· Assist in busy Medical Office
· Call, schedule and confirm patient appointments
· Enrolling patients to Essen Health Care’s program
· Assist in achieving fewer emergency room visits and hospitalizations, better coordination of services, and social service navigation
· Conduct new patient’s assessment screenings consistent with the Scope of Service
· Inform patients of Essen services and give them health education materials
· Keep patients informed of progress of scheduled appointments
· Meet with patients in their homes, community sites, Dr. office, and Health Home office
· Pitch the Health Home program to potential patients
· Conduct field visits to enroll members into the program
· Achieve a daily target of enrollments
· Complete consenting documents for all new enrollments
· Create RMA profiles for new enrollments
· Provide enrollment productivity report at the end of each day
· Field Visits Required
Job Requirements:
· High School Diploma/GED required
· Associates degree preferred
· 2-years’ experience in the healthcare, community health, social work, supportive housing, or other social service interventions
· Experience working in an Medical Office or Clinic
· Excellent customer service, organizational, interpersonal and communication skills
· Ability to multi-task and work under multiple priorities and deadlines in a fast-paced environment
· Proficiency with Word and Excel
· Bi-lingual Spanish strongly preferred
