Compensation (Based on experience and qualifications): $43,000 to $46,000 

Position purpose: This is an exciting opportunity to support Esperanza’s growing HIV services department. This position coordinates care for patients living with HIV using the AIDS Foundation of Chicago’s medical case management model.

Primary Duties and Responsibilities:

  • Perform medical case management for eligible patients with HIV/AIDS in compliance with the AIDS Foundation of Chicago, Esperanza, State, Federal, and/or health plan guidelines.
  • Co-create goals and care plans with patients in conjunction with their primary care teams, ensure understanding and appropriate follow up.
  • Work with the clinical care team to ensure patients living with HIV/AIDS have timely and coordinated access to medically appropriate levels of health and support services to ensure care continuity.
  • Address any care gaps, with input from the care team and patient, which may include (but are not limited to) managing referrals, participating in case conferences, retrieving medical records, scheduling appointments or labs, addressing clinical quality measures, and other care coordination tasks.
  • Help patients establish their medical home at Esperanza & facilitate access to benefits and/or enroll in any support services.
  • Deliver health education modules to patients, complete any required health assessments, and work with the primary care provider to follow up on any findings from the health assessments.
  • Encourage patients to become actively engaged in their own health through calm, compassionate communication, and by utilizing motivational interviewing techniques.
  • Meaningfully participate in administrative, clinical, and other team meetings.
  • Accurately enter data in to and utilize various HIV/AIDS client databases for tracking and reporting patient services when required.
  • Maintain timely and consistent communication with patients regarding their health care and resource needs.
  • Partner with other departments to help meet clinical and other operational goals in service to Esperanza’s mission and values.
  • Engage in community outreach with neighborhood schools businesses, cultural institutions, etc. to provide HIV prevention and other types of health education.
  • Other duties as assigned.

Requirements

  • Bachelor’s degree in a human service field OR an associate’s degree in a human service field with at least two years of related experience OR an associate’s degree in a non-human service field with at least 4 years of related experience.
  • Knowledge of HIV case management, HIV prevention & treatment preferred.
  • Understanding of HIV support services preferred.
  • Comfort and experience working with diverse patient populations preferred.
  • Ability to clearly communicate (verbal and written) in both English and Spanish.
  • Displays strong time management and organizational skills in a high intensity work environment.
  • Attend and successfully complete all required Esperanza and AIDS Foundation of Chicago trainings and events.
  • Follow the medical case management guidelines of the AIDS Foundation of Chicago's Northeastern Illinois HIV/AIDS Case Management Collaborative.
  • Desire to be a self-starter and take initiative, especially in a remote working environment.
  • Handle protective health information in a manner consistent with the Health Insurance Portability and Accountability Act (HIPPA).
  • Thrives working in a team-based environment.
  • Experience working in healthcare and/or a primary care setting preferred.
  • Position requires availability on evenings and weekends.
  • Demonstrates problem solving, decision-making, and sound judgment as well as customer service skills.
  • Proficient in Electronic Health Record & MS Office preferred.
  • Exceptional organizational and interpersonal skills, with attention to detail required; strong oral/written communication skills are a must.
  • Must be Bilingual in English and Spanish.
