Description

Job Summary:
The Perinatal Grant Coordinator provides overall coordination for the grant. Responsibilities include overseeing and coordinating the perinatal grant, keeping up with all data, preparing the quarterly reports, and other duties as assigned. There is an emphasis on data management and data analysis. The coordinator generates reports from Erlanger's E-chart system, EPIC to pull down quality metrics and interprets quality metric data to identify potential opportunities for improvement. There is also an emphasis on presentation, education, and explanation of data. The coordinator must express a deep understanding and knowledge of Perinatal quality metrics and reports when explaining data to nursing staff and providers. Provides training when needed for nursing staff on how to accurately capture patient quality data for metrics through face-to-face workshops and email/phone communications. Serves as subject matter expert on perinatal standards, updates, and requirements. Maintains files, records, policies and procedures as required by the Department of Health. Maintains all documentation needed to receive and maintain grant funding. Creates and maintains quarterly data for the state, Clinical Resource Specialist, and director. Performs any other duties as directed.
The perinatal coordinator reports to the Regional Perinatal coordinator which reports directly to the director of Children's Hospital.
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices' patient population. Participates in the practice's quality improvement process and performance evaluation.
Education:
Required:
Associate's Degree or 3 years in related medical field.

Minimum 2 years' work experience in physician office setting or similar community setting (may be waived at the discretion of the Regional Perinatal Coordinator). Must have an advanced proficiency in Excel, EPIC, and have excellent computer skills.

Department Position Summary:
The Perinatal Grant Coordinator will work with the regional coordinator to ensure the overall running of the perinatal grant. Assisting and coordinating meetings, data entry, data analysis, and record keeping for grant funding. Weekly monitoring of data entry, maintaining data in several databases required, and reviewing needed documentation by educators. The coordinator will review all metrics to ensure quality is being met. Will work in conjunction with regional coordinator to promote program to all stakeholders, set up and help with meetings, and performs any other duties as directed.
Primary Responsibilities:
Project Coordination
Provide logistical support to perinatal team including meeting preparation, data collection and analysis, and distribution of materials.
Communicate with the team as needed.
Conduct periodic assessment of grant projects and support educators in their implementation.
Works collaboratively with all team members to achieve goals.
Identify projects that require sustained tracking for external success.
Reporting requirements
Run all data for existing projects on a regular basis, develop creative methodologies for data dissemination; prepare and distribute monthly/quarterly summary reports to care teams and other stakeholders where needed
Work with regional coordinator to prepare and complete external reporting requirements
Obtain data from IT and other sources, analyze and summarize findings
Track, document, and submit quarterly grant data to governing bodies
Provide and monitor data, data analysis, and statistics to the perinatal grant coordinator
Perinatal Grant Implementation
Provide support to coordinators and educators and other team members in the day to day coordination of activities
Compile information, maintain detailed knowledge of each initiative, and document progress toward achieving goals
Assess implementation progress; supply qualitative and quantitative feedback to teams and individual referral providers
Prepare training materials and other learning tools for distribution to teams
Provide one on one or group coaching to staff and care teams to support information dissemination and changes
Prepare background information, develop presentations, and represent Women's at other standing meetings as needed
Work with a diverse team of staff at all levels
