Overview:

We are seeking a dedicated Home Care Nurse to join our team. The ideal candidate will have experience in critical care, catheterization, sonography, behavioral health, eClinicalWorks, vital signs monitoring, case management, nursing, ophthalmology, and ambulatory surgery center procedures.

Responsibilities:

- Provide high-quality patient care in the home setting
- Perform catheterizations and other nursing procedures as needed
- Conduct sonography and vital signs monitoring
- Manage cases effectively to ensure optimal patient outcomes
- Collaborate with interdisciplinary teams for comprehensive care
- Utilize eClinicalWorks or similar electronic health record systems
- Assist with behavioral health assessments and interventions
- Implement ophthalmology care plans as required

Experience:

- Proven experience in home care nursing or related field
- Familiarity with ambulatory surgery center protocols
- Strong background in case management and patient advocacy
- Ability to handle diverse patient populations with varying needs
- Excellent communication and interpersonal skills
- Valid nursing license and relevant certifications

Pay: Up to $45.00 per hour
