Provide services in accordance to the established POC.Evaluate and treat patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communicationCoordinate total patient care including interdisciplinary communication with other health care providers to enhance continuity of care.Guide and instruct patient and their families in prescribed therapeutic activities.Complete admission, re-certification, post hospital, and follow-up assessment procedures according to established procedures.Conduct discharge assessments and ensure appropriate discharge teaching.Qualifications:Must be a licensed Speech Language Pathologist (SLP) in the state of employmentMust have a masters or doctoral degree in speech language pathologyMust have at least one year experience as a licensed SLP.Medicare home health or hospice experience is preferred.Must possess a valid state driver license, maintain automobile liability insurance as required by law, maintain dependable transportation, and must be able to safely drive in all types of weather.Additional Information:Enhabit Home Health & Hospice is an equal opportunity employer.
