Enhabit Home Health & Hospice is Hiring a Patient Scheduler Near Springfield, IL

Schedule Hours: Monday-Friday 8:00am-5:00pm

Enhabit Home Health & Hospice is hiring a Patient Scheduler and Intake Coordinator to join our team! The Patient Scheduler and Intake Coordinator will schedule patients for field clinicians. They will communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care.

* Must have at least one year of work experience in a healthcare setting *

Responsibilities

  • Scheduling home visits for the field staff each day and assisting with the coordination of patient care.
  • Manage and complete patient schedules; assign visits timely to ensure completion of patient care, compliance with orders, and allowance for staff to appropriately prioritize visits daily.
  • Ensuring geographic scheduling to reduce drive time and to enhance staff satisfaction.
  • Accumulating admissions and intake information.
  • Communicating with physicians about orders and patient progress information.

Qualifications

  • Must possess a high school diploma or equivalent.
  • Must either 1) be a licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting; or 2) have at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services.
  • Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

  • Previous experience in home health, hospice, or pediatrics is preferred.

Requirements*

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
  • * For employees located in Oregon, requirements related to driving are not applicable unless employee has a clinical license
