Enhabit is hiring an RN to provide home health services to patients in Beverly, Peabody, and Danvers.

  • Perform in-home patient visits and ensure visit time fully meets the needs of the patient.
  • Coordinate total patient care and oversee the frequency of visits, including interdisciplinary communication with other providers, office, and physician.
  • Complete all OASIS documentation accurately, completely, and timely.
  • Perform services in accordance with physician’s orders & the established plan of care (POC). Update the POC, as needed
  • Plan, coordinate, and perform all patient functional re-assessment visits per Medicare regulations
  • Conduct discharge assessments & ensure appropriate discharge teaching
  • Teach patient and family about prescribed activities for improving independence & functionality
  • Attend weekly case conference & monthly case manager meetings
  • Must be an RN licensed in the state of employment and where practicing.
  • Must have at least two years of nursing experience.
  • Must have at least one year of experience in home health.
  • Competency with EMR platforms is required.
  • Proficiency with Homecare Homebase, both HCHB R2 (back office) and HCHB PointCare is preferred.
  • Must have strong understanding of the laws and regulatory guidelines that govern a home care operation.

Must possess a valid state driver’s license and automobile liability insurance.

Must be able to drive an automobile in all types of weather conditions.
