Note on location: Our service area is Anthem and the northwest valley. Applicant must be willing to commute to this area.

Duties:

· The social worker will contribute to the comprehensive assessment and ongoing updates to the comprehensive assessment based on the needs of the patient.

· Complete a psychosocial assessment, potential for risk of suicide and/or abuse or neglect environmental resources and obstacles to maintain patient safety, family dynamics and communication patterns, obstacles to learning or ability to participate in provision of care, obstacles to compliance with the plan of care within 5 days of admission and throughout entire duration of care.

· Evaluate and document patient and family/caregiver response to psychosocial interventions

· Evaluate caregiver’s ability to function adequately

· Evaluate need for counseling related to risk assessment for pathological grief

· Identify special needs related to cultural diversity, including communication, space, role of family members, and special traditions

· Identify support systems available to reduce stress and facilitate coping with end-of-life care

· Participate in the interdisciplinary group and address social/mental stress and/or disorder that exacerbates symptoms related to the terminal illness

· Evaluation for long-term care when appropriate and determining the patient’s and family/caregiver’s ability to accept the change in level of care

· Provide community and financial resources

· Establish funeral and end-of-life care plans, including advance directives, with the patient and their family members

· Document patient and family/caregiver response to interventions when referred to community resources and their satisfaction with services provided

· Identify bereavement needs

· Identify and assist in addressing legal concerns of the patient and family

· Offer patient, family/caregiver, interdisciplinary group member counseling as appropriate.

· Crisis intervention services for patients and families/caregivers when required, 24 hours a day, seven (7) day a week.

· Assistance with transfer of responsibilities regarding fiscal, legal, and health care decisions.

· Patient and family/caregiver questions and concerns will be promptly addressed and answered.

· All referrals, assessments, clinical notes, intervention letters and phone calls will be documented in the patient's clinical record.

· Social worker personnel will participate in hospice in-service programs to maintain and improve their specialized skills

Qualifications:

· Master’s degree in social work. Prefer medical social work (MSW)

· Minimum of 1 year hospice experience (preferred)

· Minimum of 1 year health care experience (preferred)

· CPR (preferred)

· Valid driver’s license with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Proficient computer skills with a knowledge of electronic charting
