Responsibilities :
Wanting to join a vibrant and dynamic team and to work with a well-respected company in the MD/ DC Metro.
Meets new and prospective clients (inside and outside the community) and performs an initial assessment of service needs - This could be done virtual and/or in-person, depending on client's request.
Ability to perform

Wellness checks .
Host informational

wellness sessions

for CWC clients, caregivers and other interested members.
Develops a

Plan of Care

for all new clients and updates as needed.
Communicates with care managers, caregivers, office staff, clients, family members and other medical professionals about client's needs and concerns.
Consults with the Director of Nursing regarding the supervision of services.
Provides training to the clients regarding their care, when requested.
Assures proper

maintenance of clinical records

and documentation in compliance with State and Federal laws.
Performs client & caregiver

evaluations

every 30, 90 or 120 days as needed.
Trains new staff and existing staff.
Enjoys working with a

warm

and

supportive team .
Qualifications :
RN License -

MD/VA

Compact license

OR

DC License
Current and valid

CPR / BLS

Certification
