BHI Coordinator - Douglasville

Empowering Healthcare Services is seeking a BHI Coordinator to join our team of mental health professionals. We strive for top talent and seek a talented caring professional that has broad knowledge of mental health assessments.

The BHI Coordinator will have an in-depth understanding of principles and methodologies in mental illness. They are responsible to meet with patients, gather all necessary signatures, approvals and often assessments. The BHI Coordinator will collaborate with the Empowering Healthcare Services team of physicians and other specialists to ensure a streamlined effort of treatment and/or intervention programs are implemented with a high focus on patient understanding. The client base is geriatric and based within skilled nursing facilities.

The ideal candidate will have a strong background in mental health services and a passion for helping individuals achieve emotional well-being.

**Responsibilities:**

• Monitors adherence to care plans and evaluates effectiveness
• Provide intake assessment, short-term interventions and risk assessments
• Monitors patient progress in a timely manner, and facilitates as needed
• Coordinate care that is safe, timely, effective, efficient, equitable, and client/member-centered.
• Review patient progress toward care plan goals
• Develops effective working relationships and cooperates with multiple teams throughout the case management process
• Communicate effectively with all team members
• Records and documents case information completely and accurately in EHR system
• Ensures appropriate documentation is filed promptly in patient charts
• Ensure PHI formation is secure
• Maintain accurate and up-to-date client records

**Experience:**

• Experience in social work, home and community based services, healthcare, or geriatrics preferred
• Valid Driver’s License
• Reliable Transportation
• Strong communication and interpersonal skills
• Ability to work effectively in a multidisciplinary team

Job Type: Full-time

Pay: $17.00 - $22.50 per hour

Expected hours: 25 – 32 per week
